1. Who can use your service?
A: We support disabled young people 16 to 25 years old who live in Sunderland and the surrounding areas. Parents/Carers, colleges and professionals can refer someone to us.
2. Do I need a formal diagnosis?
A:Yes - Our services are designed for people who are registered disabled and living with long - term health conditions that impact their daily life. If you're not sure whether this applies to you, feel free to reach out and we'll happily guide you.
3. Do I have to pay for your service?
Young people do not have to pay to access our service. However, employers will pay a fee to work with us.
4. How long can I recieve support?
A: Support continues until we've worked through your agreed goals. Once your goals are met, you can still access occasional check - ins if you need them.
5. Do you offer remote appointments?
A: Yes, we offer online sessions, in person sessions or a mixture of both depending on your needs.
6. I am 16 - 25 years old. Can I refer myself?
Yes! You can make a self - referral. Just complete our contact form and a member of the team will contact you in
